Personal Health Budgets – how we’ll work with you

If your local NHS team (also called Clinical Commissioning Group – or CCG) has said they’d like us to work with you on your Personal Health Budget (PHB) support plan, here’s what we’ll do:

  • Get in contact with you or your representative and arrange a time to meet
  • Talk to you and listen carefully as you or your representative tells us what healthcare and well-being needs are most important to you
  • Advise what you can and can’t include in your plan
  • Help find ways to meet your healthcare and well-being needs within the budget your local NHS team has given you
  • Work with you to put together a support plan that will keep you safe and well, and help you live the life you want
  • Send the plan we’ve agreed to your local NHS team

A Personal Health Budget gives you greater control of your healthcare – the support you want for the quality of life you want. You may choose to spend the money on services like personal care or equipment, or special therapies and activities that will help you reach your own health and well-being goals.

If you’re already happy with what you’re getting, no problem. There’s no need to change anything if you don’t want to.
